 Hohman left his rifle in his platoon's tent and was treated in the emergency room at Camp Pendleton's hospital. His rifle was moved from the tent to the site of the next training exercise, an urban-combat simulation with blank bullets, but no one checked to see whether the gun was still loaded.

On the night of the exercise, Algrim was playing the role of an insurgent. Hohman shot Algrim once in the arm and once in the head, killing him instantly, the report said. 

Okay, I don't understand this.  If they were conducting training using blanks, wouldn't the weapon have had a BFA attached?  So how does someone fire two live rounds with the BFA still on the weapon?  The first one should have caused the barrel to split, or blasted the BFA off the end of the rifle, which would certainly have made me pause before pulling the trigger again.    And how sloppy do you have to be to mix in blanks and live rounds when going on a training exercise? 

I can't help but think there is more to this story.  News Reporters don't know the difference between blank and live rounds, nor do they know what BFA's are.

If he was playing the sniper (with a bolt action sniper rifle), would he have had a blank adapter? I don't know for sure, but I doubt that he would have. I don't think our scout snipers had BFAs on the M-40's when we did ops.
They used blank rounds with MILES gear, but because they were bolt action rifles, they didn't need BFA's. (Doubt they were using MILES over there though.)
I didn't see anything about what type the weapon in question was.
It still absolutely sucks.

My guess was that he was using his M16 without a BFA so that it made a loud report. When a live round cycled he probably didn't expect it and fired the second shot totally by accident. Doesn't matter... he has to live with the guilt and both him and his chain of command will bear responsability and face the consequences of this tragic accident.
