Sham, There are no forms. You have to be Nominated By your Chapter, and signed off by your Chapter Chairman. Then you MUST have the paper work in to Manny NLT 10 June 04

Shamdog, The basic format is as follows: A letter of nomination from your chapter for National Director signed by your Chairman. With this is a letter of your intensions for the association, ie.. Increase membership , work on the raising of educational funding. Basically what YOU plan to do or achieve for the Association AS A WHOLE. Make sure you send it to the Exc. Director and in his hands NLT 10 June 04. From that you should be good to go.
